The lung tester is one of 12 reproductions of Mills 1910 Submarine lung tester made by J Papa of the National Jukebox Exchange in 2004.
They also reproduced Caille's Mickey Finn Tug of War, but in cabinet style: http://www.arcadetreasures.com/mickey-finn.html

Yes BP, Mad Magazine did steal the face, but so did Caille, proven to date at least back to 1894 according to this Wikipedia page.

Re: Unusual machines
Sega Mad Money also had him on the reels but in America, there could have been trouble for copyright reasons. An identical reel setup with $ signs to replace Alfred's funny faces was used and named "Strike a Bonanza". This machine also appeared in the UK with £ signs on the reels. 3 $ or £ symbols anywhere in the window paid 18 coins.Gameswat wrote: ↑Fri May 05, 2017 10:52 am Yes BP, Mad Magazine did steal the face, but so did Caille, proven to date at least back to 1894 according to this Wikipedia page.
