I was trolling through Google when I came across the attached PDF file.
It appears to have originated from archive work belonging to one Steve Kordek and it seems fair to credit him as described in the opening letter in the file.

That's good, it means they are in general circulation.
It can be a tad confusing when you find something with uncertain antecedents.
Then if you credit the person who last published it you can end up with a Tiger by the tail when somebody else you've never heard off starts screaming that it's theirs and nothing to do with the other person!

Sort of related, as it's a Williams of the same era, this is one I'm restoring. Someone had painted over all the lovely graphics, fortunately, I was able to get the paint off with some strong cleaner I 'liberated' from work, a few years ago.
